KnowDiabetes: A Pharmacist-led
Type 2 Diabetes Prevention and Remission Program
Historically, type 2 diabetes has been understood as a progressive condition. However, several recent studies have challenged that view. We now understand that, in some cases, progression can be stopped or slowed.
New research has shown it is possible for some people with type 2 diabetes to reduce their average glucose level to achieve an HbA1c of under 6.5% (48mmol/mol) and sustain them at that level for a prolonged period of time (at least three months) – without the need for glucose lowering medication. This is referred to as type 2 diabetes ‘remission’. (Type 2 diabetes remission 2021 – Position statement from Diabetes Australia)

The Crucial Role of Pharmacists
In the KnowDiabetes program, pharmacists play a vital role in screening and providing ongoing support for individuals who could benefit from the program.
Each participating pharmacist in the KnowDiabetes program undergoes training and assessment. This training is accredited by the Australasian College of Pharmacy (ACP) and has been accredited for 3 hours of Group Two CPD (or 6 CPD credits). It is suitable for inclusion in individual pharmacists’ CPD plans upon successful completion of the associated assessment activity under Accreditation Number: A2301KD1
Remission is possible in almost 1 in 2 adults
The KnowDiabetes program has been developed based on high-quality randomised controlled trials, which prove that with careful management, monitoring, and support, type 2 diabetes remission and prevention are possible for almost 1 in 2 adults.
1.8 million people have type 2 diabetes in Australia. Another 2 million Australians have pre-diabetes and are at high risk of developing type 2 diabetes.
The KnowDiabetes program digital tool
The program runs on a proprietary digital framework that outlines the necessary steps for healthcare professionals at each stage. This supports face-to-face interactions with pharmacists, remote telehealth sessions with dietitians and diabetes educators and information to general practice, enabling a multi-disciplinary and coordinated approach to care and support.

Very-Low-Energy-Diet (VLED*) Otherwise Known As Very-Low-Calorie-Diet (VLCD)
In the KnowDiabetes program, pharmacists receive comprehensive training on the four program phases, including how to recommend Very Low Energy Diets (VLED) and the stepped reintroduction of food.
VLEDs are medically formulated total diet replacements, designed specifically to reduce body fat stores. VLED’s provide less than 3,350 kJ (800 Calories) per day and contain sufficient essential nutrients to maintain health, including high quality protein, a small amount of carbohydrate, essential fatty acids, vitamins and minerals. VLEDs aim to maintain lean body mass whilst reducing body fat stores.

Model For Multidisciplinary Care
In a world filled with motivational videos and big talk, it’s easy to feel overwhelmed and alone. That’s why the KnowDiabetes Program prioritises human connection. The combined efforts of pharmacists, dietitians, and credentialled diabetes educators provide expert guidance, a listening ear, and a supportive shoulder. By offering valuable problem-solving skills, coping strategies, and encouragement during challenging times, we help participants build resilience and stay focused on their goals.
The Power Of Human Connection
Preventing or achieving remission of diabetes is challenging, but it is possible with the right support system. Research shows that losing just 5-7% of body weight can reduce the risk of developing type 2 diabetes by 58%. In the KnowDiabetes program, we celebrate every milestone, no matter how small, to inspire continued dedication and resilience.
